Office hours are Monday through Friday, from 8:30am until 5:00p.m.
Appointments are necessary and should be made between 8:30a.m. and 4:30p.m.
If you must cancel an appointment, please call at least 24 hours in advance.  On occasion, you may be required to wait for a time when you are here to see your doctor.  The nature of our practice dictates sophisticated testing which cannot always be planned in advance.  In addition, there are occasional emergencies.  Despite these factors, we attempt to be punctual, and respect the time of your appointment.

PLEASE ADVISE US OF YOUR NEED TO CANCEL AN APPOINTMENT AT LEAST 6 HOURS PRIOR TO YOUR APPOINTMENT TIME SO THAT WE MAY BETTER SERVE ALL OF OUR PATIENTS.

Effective January 1, 2008, there will be a $25.00 charge if we do not receive at least 6 hours advance notice for a cancelled appointment.

 Patient no-shows cause considerable hardship to our practice and patients who could otherwise be seen sooner.

You will be requested to answer a few questions.  For new patients, we require your full name, address, day & evening telephone numbers, employer and insurance information.  Please bring your insurance cards with you in order that we may keep a copy in your file.  For current patients, we request any updates or changes to the above-mentioned questions.  Minors must be accompanied by a parent/guardian.  The receptionist will give you a charge slip, which functions as your itemized statement.

Many of our patients have severe sensitivities to fragrances and their health is affected by the inhaling of scents.  Please refrain from wearing heavy colognes or perfumes when visiting our offices.

Billing and insurance questions are handled by the administrative offices at the Delaware Avenue facility.  You may call our direct lines for information during business hours.  Insurance and Billing Dept. (716) 883-6803.  If you should have a MEDICAL need after office hours, on a weekend or on a holiday, call our office at 883-6800 and the answering service will direct the call accordingly.

Payments for office examinations and procedures are requested at the time service is rendered.
Due to the fact that there are multiple insurance options available, please contact our office to see if we participate with your insurance carrier.  It is extremely important that you bring all of your insurance information, including necessary referrals and copayments to our office at the time of your visit.

Please call our office between the hours of 9:00am and 11:30am or between the hours of 2:00pm and 4:00pm during the day to have a prescription refilled.  24-hour notice is appreciated, if the situation is not an emergency.  Prescriptions over 6 months old cannot be refilled without a doctors appointment.
